Privacy Policy

How Cupertino Physical Therapy collects, uses, and protects your information.

Last Updated September 29, 2026

Your privacy matters here.

This Privacy Policy explains how Cupertino Physical Therapy, Inc. (“Cupertino Physical Therapy,” “we,” “us”) collects, uses, shares, and protects information when you visit cupertinophysicaltherapy.com, use the forms on it, or contact us through it. By using this website, you agree to the practices described here.

This policy covers the website only. Once you become a patient, the health information in your treatment records is protected under the federal Health Insurance Portability and Accountability Act (HIPAA) and California law, and is governed by our separate Notice of Privacy Practices, which you receive at your first visit and can request from our front desk at any time.

What We Collect

Information you give us.

We only collect what you choose to send us through this website:

  • Appointment requests: your name, phone number, email address, anything you write in the optional question box, and your answers about where it hurts, how long it has hurt, and how it affects your day. We also record which page you sent the request from and whether you agreed to receive text messages.
  • The 30-second Pain Check: your three answers (pain area, how long, how it affects you) and the page you were on. The Pain Check does not ask for your name or contact details, and its answers are not tied to you unless you go on to request an appointment.
  • Free benefits check: your first and last name, date of birth, ZIP code, insurance provider, member ID, group number (optional), phone number, and email address. We use these only to verify your insurance coverage and call or email you with the answer.
  • Email, phone, and job applications: anything you send us directly, including a resume or cover letter if you apply for a position.

Please do not send detailed medical history, diagnoses, or records through the website forms or by email. If you have something sensitive to share, call us at (408) 873-8188 and we will take care of it by phone or at your visit.

Collected Automatically

Cookies, analytics, and embedded content.

Like most websites, this one uses a few third-party tools that collect technical information when you visit, such as your browser type, device, approximate location based on IP address, the pages you view, and how you move through them:

  • Google Analytics (through the Google tag) measures visits and which pages and forms get used, using cookies.
  • Microsoft Clarity records how visitors scroll and click (heatmaps and session recordings) so we can find and fix confusing parts of the site. Clarity uses cookies.
  • Embedded content from Google Maps (our location and provider-network maps), YouTube (patient video testimonials, in its privacy-enhanced mode), Elfsight (our Google reviews display), and Google Fonts loads directly from those companies, which may receive your IP address and set their own cookies under their own privacy policies.
  • Our website host, Netlify, keeps standard server logs (such as IP address and pages requested) for security and reliability.

You can block or delete cookies in your browser settings, and you can opt out of Google Analytics with Google’s browser add-on. The site will still work if you do. Our website does not currently respond to “Do Not Track” browser signals, because there is no common standard for them; blocking cookies is the most reliable way to limit tracking.

How We Use It

Only to care for you and run the clinic.

  • To respond to your request, schedule your evaluation, and prepare for your first visit.
  • To verify your insurance benefits and explain your coverage and costs.
  • To send appointment confirmations, reminders, and follow-up about your care, by phone, email, or text if you have opted in.
  • To review job applications.
  • To understand how the website is used and improve it.
  • To protect the website and our patients from spam, fraud, and misuse, and to meet our legal obligations.

Text Messages

Texts only if you say yes.

If you check the text-message box on our appointment form, you agree to receive text messages from Cupertino Physical Therapy about your appointment request and your care, such as scheduling, confirmations, and reminders. Checking the box is optional and is not a condition of booking or receiving care.

Message frequency varies. Message and data rates may apply. Reply STOP at any time to opt out, or HELP for help, or call us at (408) 873-8188.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging opt-in data and consent are never shared with or sold to any third party, except the service providers that deliver our messages for us.

Sharing

We don’t sell your information.

We do not sell your personal information, and we do not share it for cross-context behavioral advertising. We share it only:

  • With the people at our clinic directly involved in your care, scheduling, and billing.
  • With service providers that operate the tools we use on our behalf, such as our website host and form processor (Netlify), our patient communication and scheduling system, our email provider, and the analytics tools described above. They may use your information only to provide those services to us.
  • With your insurance company, when you ask us to verify your benefits or when we bill for your care.
  • When required by law, to protect someone’s safety, or as part of a sale or transfer of the practice, in which case this policy would continue to apply to your information.

Security & Retention

How we protect it and how long we keep it.

This website is served only over an encrypted (HTTPS) connection, and form submissions go only to our clinic and the service providers described above, with access limited to the people who need it. No method of sending or storing information online is perfectly secure, so we cannot guarantee absolute security, but we take reasonable steps to protect what you share.

We keep website form submissions only as long as we need them to respond to you, schedule and support your care, and meet our legal and record-keeping obligations. Information that becomes part of your patient record is kept for the period California law requires for medical records.

Your Choices

Your rights.

You can ask us at any time to tell you what personal information we have collected about you through this website, to correct it, or to delete it, and you can opt out of texts or emails from us. California residents may have additional rights under state privacy laws, and we will honor them where they apply. We will not treat you differently for making a request. To make a request, call (408) 873-8188 or email cpt95129@gmail.com. We may need to confirm your identity before acting on it, and some information may need to be kept to meet legal or medical-record requirements.

Children

Parents book for kids.

This website is meant for adults. We do not knowingly collect personal information online from children under 13. If your child needs care, please submit the request yourself or call us. If you believe a child has sent us information, contact us and we will delete it.

Changes & Contact

Questions? Ask a real person.

We may update this policy as our website or the law changes. When we do, we will change the “Last Updated” date at the top of this page.
